He now works as a doctor on call for one of the big banks providing staff healthcare.

I suggest we subcontract more services out to Wetherspoons. Pay benefits direct to Tim and in return he waters the masses with cheap beer and feeds them a meal a day.
Maybe set up local medical services and the benefits office on site and save the travel time.
