I think it needs proper diagnosis by trained professionals rather than slurs by Trump or anyone supporting him.

Biden clearly has mobility issues, and is getting tired more quickly. As someone who turns 80 this week I can understand that, but if anyone suggested that meant I am suffering cognitive decline they would quickly be shown how wrong they were.

How do you know he has refused?

Such matters quite rightly remain confidential between a client and his doctors.

Ok, I found the original report which makes for interesting reading, telling a very different story to the one that the Republicans and the right wing media want you to believe. I don’t though, not on first reading, find the phrase you referred to.

Not that Trump is racist, sexist (not to mention crook, traitor, liar, etc etc) of course.

If anyone seriously doubts Biden has cognitive decline, rather than just saying so to be a contrarian, I would suggest they are in the same deluded condition as Biden.

That being said, when you look at the various compilations of Harris' gaffes and word salads, it's not that different from Biden.
Confusing North and South Korea for example.

Responsibility Deficit Disorder is a medical condition. Symptoms include inability to be corrected when wrong, false sense of superiority, desire to share personal info no else cares about, general hubris. It's a medical issue rather than pure arrogance.

His doctors said he doesn’t need one! Not that he refused! Why on earth would he pander to his enemies rather than listen to his doctors? I know I wouldn’t.

Then “they”, and you it seems, are being mega complacent. It was always close between Trump and Biden anyway. This change has the potential to tip the balance and anyone not recognising that is in denial.
