I would say the letters are nearer 30p each to produce and mail and could be down to GDPR that this hasn't been sent as a text or email as they would need your consent to do so (ie, you can't be automatically opted in)

High cost appointments are usually followed up with phone calls the day before (MRI Scan appointments).

Problem with NHS these days is not the cost but the perceived value and the service - have a crap experience and the cost looks high, have a great experience and the cost isn't thought about. It's a people service and unfortunately has been subject to change and outsourcing whereby many NHS roles are now recruited for / contracted and treated as (low paid) jobs where they once were vocations.

30p would be accurate if it was 2nd class but OP said they are 1st class. You'd be looking at around 60p for dual sided mono for that, even at their volumes and through CCS.
