It's simply a matter that as we reduce some of the diseases and accidents that affect life expectancy others come into play, so no more cholera, TB and smallpox means new diseases of old age: cancer, dementia and arthritis. In addition advances in medical science mean that conditions that used to be mysterious (and so blamed on God, demons, or whatever) are recognised and explained. Get rid of those and others will come into play, etc.
